Supporting patients and clients to be healthier can be a challenge. To help address Nova Scotia’s high rates of cardiovascular and chronic disease, it’s important that individuals have access to high quality health care and rehabilitation programs, and that they know how and where to find the information and resources when they are needed. Health professionals also require access to information that can help support their health promotion efforts.
The Heart&Stroke Health Directory is an online resource that provides information on key resources available in Nova Scotia communities to help people be active and healthy. Re-launched in June 2008 and sponsored by Pfizer Canada, the Health Directory has a new look, providing more user-friendly access to community supports and is a gateway to information on key resources that health care providers can use to help patients be more active and healthy.
Whether used as a preventative resource for access to early care or as a rehabilitation tool following a critical event such as heart attack or stroke, health care providers can use the Health Directory as a referral for patients looking for supports on physical activity programs, healthy eating, heart disease, stroke management and more.
